29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about August 19, 2021, a roofer was using a pneumatic nail gun to install shingles without wearing appropriate eye protection, exposing the roofer to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about August 19, 2021, and at times prior thereto, two employees were installing shingles to the roof of the residential home under new construction up to approximately 18 feet high without fall protection, exposing the workers to a fall hazard.

On or about August 19, 2021, and at times prior thereto, two employees were installing shingles without having fall protection training, exposing the employees to a fall hazard.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about August 19, 2021, employees accessed the roof of a one-story home by using a portable ladder that was not extended at least 3 feet above the upper landing surface, exposing the roofers to a fall hazard of approximately 18 feet to the ground below.
