29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system. On 16 April 2014, two workers were performing roofing activities on a low sloped roof of a subdivision entry tower without fall protection, exposing the workers to a fall hazard of approximately 30 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On 16 April 2014, two workers gained access to a tower rooftop by means of a 32 foot aluminum extension ladder that was not extended at least 3 feet beyond the roof edge, exposing the workers to a fall hazard of approximately 30 feet to the ground below.
