29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by nip points: On or about February 24, 2014, and times prior to, at 1801 North Lincoln Street in Street in Siloam Springs, Arkansas, in the QMT Cell 4510 Builder area, guarding was not provided on the builder to prevent employees from contact with the rotating mandrel and pressure roller. Employees were exposed to the hazard of contact with nip points.
