On and before April 16th, 2014, the employer did not ensure that Vertical Metal Band Saw, Hyd-Mech, Model V-18, Serial No. No. J1297522, used in the Fabrication Metal Shop, was equipped with adequate point-of-operation blade guarding. The unused portion of the blade was not guarded to protect employees against amputation hazards during operating cycles.    (b)Ferrie Franzmann Industries @ 1505 W. 3rd Street, Denver, CO. 80223.  On and before April 16th, 2014, the employer did not ensure that Horizontal Metal Band Saw, FMB Model Phoenix, Serial No. No. NG0111O, used in the Fabrication Metal Shop, was equipped with adequate point-of-operation blade guarding. The unused portion of the blade was not guarded to protect employees against amputation hazards during operating cycles.
