29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Location: 4753 Dixie Hwy, Fairfield, OH: On or about April 21, 2014 an employee working alongside his employer was observed removing an EFIS from a commercial building while inside a Genie S-40 Telescopic Boomlift approximately 12 foot above ground level without a body belt being attached to the boom or basket.
