OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing and struck-by hazards while using automotive lifts: (a) On or about April 3, 2014, and times prior to that date, employees were exposed to potential crushing hazards while using the Rotary in-ground automotive lift in Bay 6, in that the lift was not lowered on to the safety locks or the vehicle was not supported with suitable jack stands rated for the intended load. Abatement Note: Among other methods, several feasible and acceptable abatement method to correct the hazards is to implement the use, inspection, and maintenance requirements of the ANSI-ALI ALOIM: 2008, Standard for Automotive Lifts-Safety Requirements for the Operation, Inspection and Maintenance.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads: (a) On or about April 3, 2014, and times prior to that date, employees who worked around and under the Rotary in ground lifts were exposed to injury in that lifts were not being lowered onto the safety locks when elevated to perform work in the shop area of Pep Boys Store # 0242. Pep Boys was previously cited for a violation of the OSH ACT of 1970 Section (5)(a)(1) which was contained in OSHA inspection 914663 Citation 1 Item 1, issued on July 10, 2013 and became a final order on August 2, 2013 with respect to a worksite located at 5134 Wadsworth Blvd., Lakewood, CO 80123. Pep Boys was previously cited for a violation of the OSH ACT of 1970 Section (5)(a)(1) which was contained in OSHA inspection 923530 Citation 1 Item 1, issued on August 7, 2013, and became a final order on August 26, 2013 with respect to a worksite located at 7625 N. Academy Blvd., Colorado Springs, CO 80920. Pep Boys was previously cited for a violation of the OSH ACT of 1970 Section (5)(a)(1) which was contained in OSHA inspection 956913 Citation 1 Item 1, issued on February 7, 2014 and became a final order on February 21, 2014 with respect to a worksite located at 135 N. Academy Blvd., Colorado Springs, CO 80909. Abatement Note: Among other methods, several feasible and acceptable methods of abatement would be to follow the requirements of ANSI/ALI ALOIM (Standard for Automotive Lifts- Safety Requirements for Operation, Inspection and Maintenance) and "Lifting it Right- A Safety Manual from the Automotive Lift Institute" which include, among other requirements, the following: 1. Lowering the lifted vehicle onto the manufacturer's safety locks and/or suitable jack stands rated for the intended load. 2. Provide training to employees on proper use and care of the lifts.
