29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:      a) 205 Meister Ave., North Branch, NJ worksite - The employer did not provide a continuing, effective Hearing Conservation Program for employees exposed to noise levels above 85 dBA.     Production Line #1 & #2 - Employees were exposed to continuous noise levels of 88.8 dBA for an 8-hour Time Weighted Average (TWA), which is equivalent to a dose of 85.7%. Sampling time was 473 minutes.     Zero exposure was assumed for all un-sampled time periods.      Condition noted on or about 5/8/2014.

The overhead piping sagged in the middle as a result of the defective anchor points.     Condition noted on or about 4/22/14.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts of conveyors and rotating parts:    205 Meister Ave., North Branch, NJ worksite: Guards were not provided where employees could be struck by or caught in machinery.    a) Gas House Lines #1 & #2 - The gasser heads were unguarded allowing employees to inadvertently be cut or crushed by the vertical movement.      b) Gas House Line #1 & #2 -  Beneath the gasser head, moving parts of the equipment including but not limited to conveyors were exposed which could lead to employees inadvertently being caught in the moving parts.    c) Filling Room Line #1 - Moving parts of the conveyor systems including but not limited to the conveyor belt were not guarded to prevent employees clothing and/or hair from accidentally being caught in the in-running nip points during operation.    d) Mechanic Shop - The Verticut band saw was not provided a guard for the unused portion of the blade.    Condition noted on or about 4/22/14.
