29 CFR 1910.23(b)(9): Ladders were not inspected before the initial use in each work shift, or more frequently as necessary, to identify any visible defects that could cause injury:    a) American Spraytech, LLC/205 Meister Ave Branchburg, NJ- Production area: The employer did not ensure that movable Cotterman ladder was inspected before the initial use in each work shift. The ladder had visible defects as it was bent by the base making it unsafe for employees to use and exposing them to fall hazards.    Condition observed on or about October 10, 2018.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) American Spraytech, LLC/205 Meister Ave Branchburg, NJ- Production area: The employer did not develop and implement machine specific energy control procedures to include specific steps for shutting down, isolating, blocking and securing machines. Employees were required to perform maintenance activities  on equipment used in production lines #1, #2 and #3, and were exposed to hazardous energy stored within the system, including: electrical, mechanical, chemical, and thermal.    Condition observed on or about October 10, 2018.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) American Spraytech, LLC/205 Meister Ave Branchburg, NJ- Production area: The employer did not provide hazardous chemical training for employees working in area where flammable gases, hydrocarbons and Dimethyl ether are used. Employees were not provided with the information required by the Hazard Communication Standard including but not limited to the information on and the location of the Safety Data Sheets.     Condition observed on or about October 10, 2018.
