The ladder had visible defects as it was bent by the base making it unsafe for employees to use and exposing them to fall hazards. Condition observed on or about October 10, 2018.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) American Spraytech, LLC/205 Meister Ave Branchburg, NJ- Production area: The employer did not develop and implement machine specific energy control procedures to include specific steps for shutting down, isolating, blocking and securing machines. Employees were required to perform maintenance activities on equipment used in production lines #1, #2 and #3, and were exposed to hazardous energy stored within the system, including: electrical, mechanical, chemical, and thermal. Condition observed on or about October 10, 2018.
