29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a) As observed on or about 04/10/2014, at the construction of a residence at 14249 Alistar Manor Drive Ayerworth Glen in Wimauma (FL) 33598.  Several planks used in the tubular welded scaffold system used to apply stucco were approximately 8-inches in width and exposed employees to fall hazards of about 13-feet.

29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6-inches (15 cm):  a) As observed on or about 04/10/2014, at the construction of a residence at 14249 Alistar Manor Drive Ayerworth Glen in Wimauma (FL) 33598.  The southwest plank of a tubular welded scaffold system used to apply stucco extended only two inches over the centerline of its support and exposed employees to fall hazards of about 13-feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a) As observed on or about 04/10/2014, at the construction of a residence at 14249 Alistar Manor Drive Ayerworth Glen in Wimauma (FL) 33598.  Crossbraces were used as a means of access to a tubular welded scaffold system used to apply stucco exposing employees to fall hazards of about 13 feet.

A tubular welded scaffold system used to apply stucco at the construction site was used without guardrails and exposed employees to fall hazards of about 13-feet.

Employees were observed standing on the shed roof while applying stucco at the construction site.  Employees were exposed to fall hazards of about 13-feet.
