29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) For employees working at 20230 Tessaro Drive #213 in North Port, as observed on or about 29 May, 2014. Employees were observed installing plywood sheathing on a roof with a 5:12 pitch. The eave to ground height was approximately 10 feet and the employees had no fall protection system in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) For employees working at 20230 Tessaro Drive in North Port, as observed on or about 29 may, 2014. Employees using an extension ladder to access the upper landing surface did not have the ladder extended far enough above the upper landing surface. The extension ladder only extended approximately 12 inches above the upper landing surface.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) For employees working at 20230 Tessaro Drive in North Port, as observed on or about 29 may, 2014. Employees were using power tools that were supplied power through extension cords that were missing strain relief. The outer insulation on the extension cords in use which provides strain relief was pulled away from the plug end.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a) For employees working at 20230 Tessaro Drive in North Port, as observed on or about 29 May, 2014. Employees were using relocatable power taps to supply power to the power tools in operation. Two relocatable power taps were in use to increase the number of available outlets on the branch circuit.

Employees were using power tools that were supplied power through extension cords that were missing the ground prongs. The extension cords in use did not have a permanent and continuous path to ground.
