In department 2, core room, the employer did not ensure that lock out procedures gave specific procedural steps for blocking and bleeding the hydraulic pressure on the EMI blower machine.

29 CFR 1910.147(c)(7)(iv): The employer shall certify that employee training has been accomplished and is being kept up to date. The certification shall contain each employee's name and dates of training:    a.  The Respondent did not maintain records certifying that authorized employees in the Core Room, Department 2, received required training for energy isolation and control, before operating the EMI blower, which required lockout/tag out procedures to be followed during daily operations.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. In the core room, department 2, the employer did not ensure guards were in place at the point of operation on the EMI blower.    The Columbus Steel Castings Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 313148207, citation number 2, item number 2 and was affirmed as a final order on November 25, 2010, with respect to a workplace located at 2211 Parsons Ave, Columbus, OH 43207.
