Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: Location: 1361 Tennessee Ave, Cincinnati, OH: On or about April 23, 2014, one employees working for M & S Roofing, LLC was observed working by the edge of a commercial roof more than 13 feet above the ground below without the use of any fall protection.
