29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about August 31, 2015 employees were performing low-slope associated roofing work on an approximate 98 x 83 commercial structure roof deck on which located in a central area of that roof deck employee(s) were exposed to an approximate 36"X43" metal roof deck hole which was not adequately protected exposing employee(s) to fall potentials in excess of 15 feet.

29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge.   On or about August 31, 2015 and prior to that time employees were performing low-slope associated roofing work on an approximate 98 x 83 commercial structure roof deck with unprotected sides or edges without compliant conventional fall protection systems and along three sides of the roof deck structure had the warning line systems erected less than one foot from the unprotected side or edge and along the front of the roof deck structure warning lines were erected as close as 16 inches from the unprotected side or edge exposing employees to fall potentials in excess of 15 feet on all sides of the structure.

29 CFR 1926.502(j)(7)(i): Materials and equipment shall not be stored within 6 feet (1.8 m) of a roof edge unless guardrails are erected at the edge.   On or about August 31, 2015 and prior to that time employees were performing low-slope associated roofing work on an approximate 98 x 83 commercial structure roof deck with unprotected sides or edges and at ground level without compliant material storage, in that located along one side area of the roof deck there were multiple bundles of ValuTherm insulation stored at a range of 2 10 to 4 from an unprotected side or edge of the roof deck.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   On or about August 31, 2015 and prior to that time employee(s) performing low-slope associated roofing work on an approximate 98 x 83 commercial structure roof deck with unprotected sides or edges without conventional fall protection systems and non-compliant warning lines, material storage and roof hole(s) exposed to fall hazards or falling material struck by hazards in excess of 15 feet who were not trained or knowledgeable of the procedures to be followed to minimize those hazards.
