29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signaled that it is safe to do so: a. At the yard area, employees were exposed to the hazard of being run over by a bulldozer, in that, the Komatsu FD-31PX bulldozer was not provided with an audible back-up alarm and a spotter was not used when the equipment was operated in reverse.
