29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) On or about April 23, 2014, a forklift driver working in the Shipping and Receiving Department was operating a Yale propane forklift. His last forklift training and performance evaluation was March 8, 2011 (more than 3 years since his last evaluation).

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: (a) Wand type compressed air guns were used to blow off sawdust on the router tables. The wands were not equipped with the venting hole to prevent pressure build-up to less than 30 p.s.i. when the nozzle tip is blocked.
