29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) Livingston Street, Philadelphia, PA: While employees were performing bricklaying on the front of the building, the employer did not ensure the work area was kept free from wooden pallets, nails and other construction debris beneath and around the tubular welded frame scaffold. This was observed on April 23, 2014.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:  a) Livingston Street, Philadelphia, PA: Employees performing bricklaying from the second level of a tubular welded frame scaffold were exposed to fall hazards of approximately twelve (12) feet. The employer did not ensure that employees were protected from falls as a guardrail system was not installed along the open ends of the platform from which the employees were working. This was observed on April 23, 2014.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) 2999 Livingston Street, Philadelphia, PA. - The employees who were working on the tubular welded frame scaffold approximately 16 feet above grade were not provided training by their employer or by a person qualified to recognize hazards associated with tubular welded frame scaffolding as the second scaffold tier was constructed without end rails and one of the legs was missing a baseplate. This was observed on April 23, 2014.

This was observed on April 23, 2014.
