Carlisle St. - The employer did not ensure that the working level of the tubular welded frame scaffold was fully planked. The employees were hoisting and carrying five (5) - gallon buckets of stucco from two and three planks on top of the 5th level of scaffold and were exposed to a fall hazard of approximately thirty-two (32) feet. Observed on or about 6/24/14.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. a) Jobsite, 1526 N. Carlisle St. - The scaffold planks on the working level of the tubular welded frame scaffold were spaced up to approximately one (1) foot apart and up to approximately one (1) foot from the uprights, exposing employees who were hoisting and carrying five (5) - gallon buckets of stucco to a fall hazard of approximately thirty-two (32) feet. Observed on or about 6/24/14.

Carlisle St. - Employees were exposed to a collapse hazard while performing stucco operations from the top of the 5th level of a tubular welded frame scaffold which was not restrained by guys, ties and braces. The height of the scaffold was approximately thirty two (32) feet, the base width was approximately five (5) feet, and the height to base width ratio was approximately six to one (6:1). Observed on or about 6/24/14.
