29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Gen-Rubber LLC, Galion, Ohio: On or about September 17, 2014, the employer did not ensure that the auger on the Granulator #1 screw conveyor was guarded to protect employees from crushing and amputation hazards. b) Gen-Rubber LLC, Galion, Ohio: On or about September 16, 2014, the employer did not ensure that the auger on the Veco screw conveyor, located near Granulator #1, was guarded to protect employees from crushing and amputation hazards.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) Gen-Rubber LLC, Galion, Ohio: On or about April 16, 2014, the employer did not ensure that the rotating drive pulley and belt, mounted on the hopper which feed the granulator #1 screw conveyor, was properly guarded. Employees were exposed to amputation hazards if they were to inadvertently contact the belt and pulley nip point.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Gen-Rubber LLC, Galion, Ohio: On or about September 17, 2014, the employer did not ensure that electrical equipment utilized by employees was being used as listed, in that a relocatable power tap (110 volts) was being used in the granulator area to power a water pump. The relocatable power tap was brown and melted where the pump was plugged in, exposing employees to burn hazards in the event of a fire.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) Gen-Rubber LLC, Galion, Ohio: On or about April 16, 2014, the employer did not ensure that the unused opening in the 220 volt AC panelboard cabinet, located near granulator #1 was effectively closed to prevent damage to energized circuits. Inside of the panelboard, there was a circuit breaker missing, creating an opening to energized circuit, exposing employees to shock and fire hazards.
