29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: a) 204 Applegarth Road, Building #14, Monroe Twp., NJ 08831. Two employees were installing new roof sheathing using a pneumatic nail gun while not wearing any eye and or face protection exposing them to eye and face injuries from flying debris. Condition occurred on or about 04/22/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 204 Applegarth Road, Building #14, Monroe Twp., NJ 08831. Two employees were installing new roof sheathing materials with no fall protection systems in place exposing them to fall hazards. Condition occurred on or about 02/22/14.
