29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) 16 Cypress Street, Carteret, NJ 07008: Employee was observed performing ground clean-up operations, with overhead roofing work being performed, without a protective helmet, exposing him to struck-by hazards.      Condition observed on or about April 24, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:       a) 16 Cypress Street, Carteret, NJ 07008: Employees were observed performing roofing operations at a height of approximately 23 feet to the ground below with no fall protection in place, exposing him to fall hazards.      Condition observed on or about April 24, 2014.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:     a) 16 Cypress Street, Carteret, NJ 07008: Employees were allowed to climb the rungs of a Reimans & Georger Corp (RGC) Model P225 Material Hoist to access the roof.  The RGC P225 Platform Hoist Instructions state: "NEVER climb the track; use a ladder."       Condition occurred on or about April 24, 2014.
