Storage racks, storing various parts and materials including, but not limited to spare machine parts, fans, and maintenance type items were not secured to prevent the system from moving if struck by forklifts over time.  Employees are exposed to struck-by hazards.

29 CFR 1910.213(h)(4): Radial saw(s) were not installed in a manner so as to cause the cutting head to return gently to the starting position when released by the operator:     The employer is failing to protect employees from laceration hazards by not ensuring the equipment is properly set up so the cutting head of the radial arm saw always returns to the starting position.  When extended, the cutting head would not return to the starting position.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     The employer is failing to ensure employees are protected from electrical hazards.  Employees are working with an electrical outlets box in the immediate vicinity of the reverse osmosis equipment located east of the Positive Drive cell which is not being used in accordance with its listing and labeling.  The outlet box is attached to the end of a flexible cord to form an extension cord which has 2 plugs.  The type of box being used is designed for use in an enclosed wall.  It is not designed to withstand the damage and use as a portable box.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:     The employer is failing to ensure that the areas in front of circuit breaker panel boxes are free from obstruction and able to be readily accessed.  The panel box located in the "Dallas Palace" was blocked by numerous pieces of furniture, equipment and materials.  This impedes access to the box in an emergency situation in which the electricity would need to be turned off at the box.
