29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):    On or about April 22, 2014 and times prior thereto, located at the Ray Corcoran Gas Unit Well#1:    (a) No signage was present for piping, vessels and tanks that are protected from very high pressure gas by a valve at a remote location.  Signs are needed to inform workers at the site of the active well that is remotely located from the from the tanks and vessels.    (b)  Pipes and valves were not identified as to their purpose.  The flare line and valve were not identified.   This condition created a hazard of closing the flare line when it needed to be open to relieve excess pressure. Improper valve operation could cause catastrophic failure of the separator tank resulting in serious physical injury or death.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about April 22, 2014 located at the Ray Corcoran Gas Unit Well#1 workers installed a ball valve in a line extending from a separator tank without first ensuring that the high pressure wellhead was locked in the closed position.  This condition exposed the workers to the hazards of fire and being struck by shrapnel in the event of a pressure release or catastrophic overpressure of the separator.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:    On or about April 22, 2014 and in times prior thereto the employer at the Ray Corcoran Gas unit Well #1 well site did not provide effective training in the emergency procedures to evacuate immediately when leaking flammable chemical under pressure was detected, exposing employees to the hazards of fire and chemical splashes.
