29 CFR 1910.106(d)(2)(i): Approved containers and/or portable tanks were not used for the storage of flammable liquids: On or about January 6, 2016 and in times prior thereto a plastic container designed for use at race tracks only was used to store gasoline. The gasoline was stored for use in the gasoline engine mounted with the air compressor. This container was a fire hazard in that it was not approved for this use and this warning was molded into the side of the container. The container was located in back of a pickup truck along with other oil field equipment at the worksite.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about January 6, 2016 and in times prior thereto the valve located between the separator and the pressure relief valve was not locked in the open position. The valve was located on Sundown energy LP lease #264388. This condition created an explosion hazard in the event someone accidently closed the valve and pressure built up in the separator.
