29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition:     a) Front, south side, of residence located at 16447 Waxmyrtle Road, Milton, Georgia. The employer did not provide the employees personal protective equipment, impervious gloves and protective eye wear, to prevent contact with Portland cement, a component of masons cement.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:     a) Front, south side, of residence located at 16447 Waxmyrtle Road, Milton, Georgia. Employees were not protected from falling approximately 29-feet 7-inches or 24-feet 2-inches, from the fabricated frame scaffold working platforms, to the ground by the use of a guardrail or safety net system or personal fall arrest systems.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling.     a) Front, south side, of residence located at 16447 Waxmyrtle Road, Milton, Georgia. An employee working below an active scaffold working platform was not protected from being struck by fall objects by a hard hat or the installation of toe boards along the edge of the overhead working platforms.

29 CFR 1910.1200(e)(1): The employer did not develop, implement and maintain at the workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:   a) Front, south side, of residence located 16447 Waxmyrtle Road, Milton, Georgia. The employer did not develop, implement, or maintain a written hazard communication program which at least describes forms of warning, material safety data sheets, and employee information and training as it pertains to, in this case, Portland Cement, a component part of masons cement.
