29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:   The employer does not ensure that scaffold legs bear on base plates and mud sills.  This violation was observed on or about April 28, 2014, at the job site where employees were exposed to a fall hazard when working on an elevated pump jack scaffold without base plates and mud sills.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffold legs bear on base plates and mud sills.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   The employer does not protect employees from falling when working on scaffold more than 10 feet above a lower level.  This violation was observed on or about April 28, 2014, at the job site where employees were exposed to a fall hazard when working on an elevated pump jack scaffold at 17 feet above the ground without being protected from falling to the ground.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees working from a scaffold 10 feet or more above a lower level are protected from falling.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage:   The employer does not ensure that all brackets and braces are fabricated from metal plates and angles.  This violation was observed on or about April 28, 2014, at the job site where employees were exposed to a fall hazard when working on an elevated pump jack scaffold without the scaffold braces having been fabricated from metal.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all brackets and braces are fabricated from metal plates and angles.

This violation was observed on or about April 28, 2014, at the job site where employees were exposed to a fall hazard when working on an elevated pump jack scaffold without the scaffold being secured at the bottom and other points as necessary.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure scaffold poles are secured to the structure by rigid triangular bracing at the bottom, top, and other points as necessary.
