Floor holes were left unprotected along the walkway that the slab handler for the round blade head saw used. (b) At American Walnut LLC on or about April 28, 2014 workers were exposed to stepping into floor holes. Floor holes were left unprotected on the deck used by the log handler ahead of both head saws. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.24(f): All treads shall be reasonably slip-resistant and the nosings shall be of nonslip finish. Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: (a) At American Walnut LLC on or about April 28, 2104 the rise height of steps leading to a walkway over a moving conveyor were not of uniform height. (b) At American Walnut LLC on or about April 28, 2014 the rise heights of steps leading from the main floor to a basement area near the circular head saw were not of uniform height. (c) At American Walnut LLC on or about April 28, 2014 the tread depth of steps leading to the work platform for the slab handler at the band head saw were not uniform throughout the flight of stairs. (d) At American Walnut LLC on or about April 28, 2014 the treads on steps leading to a walkway over a moving conveyor were not reasonably slip-resistant. (e) At American Walnut LLC on or about April 28, 2014 the treads on steps between different working levels near the circular head saw were not reasonably slip-resistant. The steps did not have any nosing to protect the tread from moving or being slip-resistant. (f) At American Walnut LLC on or about April 28, 2014 the steps leading from the log yard area to the log loading platform at the head saw did not have consistent tread height and tread depth.

The controls for the hoist were not protected from the elements of the weather including rain.
