29 CFR 1910.157(g)(2): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer did not also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting upon initial employment and at least annually thereafter:  (a) At the establishment:  Employees did not receive at least annually an educational program to familiarize them with the principles of fire extinguisher use and the hazards of fighting small fires. The employer provided fire extinguishers and employees understood they were to use the extinguishers to fight small fires. There were small batteries, improperly wire 110 VAC electrical equipment, and paper in the facility. Employees were exposed to the hazard of fire.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  (a) In the storage shed above the work bench:  Small, multiple outlet, surge protection bars, intended for small electronic equipment, were used to power two, four feet long, 110 VAC florescent light fixtures, exposing employees to the hazard of contacting electricity.  (b) In the storage shed  An octagon shaped, metal electrical box, that contained a 110 VAC receptacle that was powered with a flexible cord connected to the box, was used as a multiple outlet extension cord while conducting field work. The box was designed to be mounted to a surface and powered with permanent wiring. Employees were exposed to the hazard of contacting electricity.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion:  (a)In the storage shed above the work bench:  There were no connectors, bushings or rounded surfaces to protect flexible cords that entering the sharped edged openings in the housings for two, 110 VAC, four foot long florescent light fixtures, exposing employees to the hazard of contacting electricity.
