29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: Jobsite located at 64 South Street, West Hartford, Connecticut: On, or about, April 28, 2014, employees who were installing a shingled roof were not protected from falling through skylight openings installed on the roof surface.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Jobsite located at 64 South Street, West Hartford, Connecticut: On, or about, April 28, 2014, employees who were installing a shingled roof were not using personal fall arrest equipment. The employees were exposed to a fall distance of eleven feet, three inches to the ground below.
