29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed. a) Jobsite: Ground level inside demolished building - On or about 4/29/14, an engineering survey was not completed by a competent person prior to permitting employees to conduct demolition operations on a 3 story row home building. During the demolition operations, 2 employees were struck by a section of slate that fell from side of the adjoining building that was not braced.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. a) Jobsite: Ground level inside demolished building - On or about 4/29/14, Gama Wrecking did not have a competent person conduct frequent and regular inspections of the job site, materials, and equipment, to recognize, evaluate, and control hazards present during demolition activities.
