29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e): a) 930 S. 11th St. Philadelphia, PA 19147 - The employer did not ensure that an employee who was in the basket of an aerial lift wore a personal fall arrest system and lanyard attached to the boom or basket, thus exposing the employee to fall hazards. This condition was observed on April 9, 2018.

Gama Wrecking, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 974960, citation number 1, item number 1, group a, and was affirmed as a final order on October 29, 2014, with respect to a workplace located at 3026 W. Diamond St., Philadelphia, PA 19121.
