On April 30, 2014, the exit from the office break room in the direction toward the back of the store (opposite to the changing rooms) was obstructed by a product display stand that limited egress to 11 inches.

On April 30, 2014, the electric circuit breaker panel in the office break room was missing four switch panel plugs in slots 3, 9, 10 and 12. b. On April 30, 2014, the outlet box beneath the stereo equipment in the office break room was missing a knock out seal to cover the exposed hole. c. On April 30, 2014, the outlet box behind the cash register near the entrance to the store was missing a knock out seal to cover the exposed hole.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a. On April 30, 2014, access to the electric circuit breaker panel facing the front of the room was blocked by a built in table that protruded 33 inches from the wall in front of the panel. b. On April 30, 2014, access to the electric circuit breaker panel facing the back of the room was blocked by a built in table that protruded 33 inches from the wall in front of the panel.
