R-22 Engine Room, Skid Package #1, Oil Pot, Three-way Valve Manifold - The employer did not utilize  written procedures that specify the equipment that should be shut down and isolated as well as the precautions to be followed before the removal of the pressure relief valves. Contractor employees were exposed to the unexpected release of ammonia while removing these valves.  Observed on or about April 29, 2014.
