29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 04/30/2014, at the jobsite at 8282 Forsyth Blvd, St Louis, MO, employees engaged in roof repair/installation operations were not provided conventional fall protection. Harnesses were worn by employees; however, they were not all attached to an anchor point. Eave to ground was approximately 9 feet.
