29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    At the jobsite located 2816 Shenandoah Avenue, Saint Louis, Missouri , Innovative Construction and Roofing did not initiate and maintain programs that provide for frequent and regular inspections for its subcontractor that was installing shingles with a pneumatic nail gun on the portico of a residential porch.  The foreman was onsite and observed the employee(s) working without fall protection and eye protection. The following hazards were noted on the job site:  a/  Lack of fall protection  b/  Lack of eye protection    Innovative Construction and Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1111037, citation number 1, item number 1 and was affirmed as a final order on January 12, 2016, with respect to a workplace located  401 Mar-Le Drive, Wentzville, Missouri  63885.    Innovative Construction and Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1050259, citation number 1, item number 1 and was affirmed as a final order on May 20, 2015, with respect to a workplace located at 2320 Hampton Avenue, Saint Louis, Missouri  63139.
