29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck was dismounted and within 25 ft. of the truck still in the operators view, the load engaging means (forks) were not fully lowered, controls neutralized, and the brakes set to prevent movement: a) South side of plant 3: The forks of a Caterpillar telehandler were not lower to the ground before the operator left the cab of the telehandler. The forks were supporting an unsecured 5000 lb., 50 foot long beam. Note) Abatement certification AND documentation ARE required for this item.

29 CFR 1910.178(o)(1): Stable or safely arranged loads were not handled: a) South side of plant 3: An unsecured, 50 foot beam was centered on the elevated forks of a Caterpillar powered industrial truck (telehandler). The act of attempting to flip the load 180 degrees while on the elevated forks, caused it to be un-stable. Note) Abatement documentation AND certification ARE required for this item.
