29 CFR 1926.451(g)(1): The employer did not ensure that each employee on a scaffold more than 10 feet (3.1 m) above a lower level was protected from falling to that lower level. On or about April 29, 2014 and at times prior to, employees working on a single story commercial building were exposed to falls of approximately 12 feet to the concrete below. Some areas were not equipped with fall protection.

29 CFR 1926.1053(b)(4): The employer did not ensure that ladders were used only for the purpose for which they were designed. On or about April 29, 2014 and at times prior to, employees working on a single story commercial building were exposed to falls of approximately 12 feet to the concrete below while using a step ladder inappropriately. Ladder was not used in accordance with the manufacturer's design.
