29 CFR 1910.1026(l)(1)(iii): The employer did not include chromium (VI) in the hazard communication program established to comply with the Hazard Communication Standard (HCS), ensure that each employee had access to labels on containers of chromium (VI) and to safety data sheets, and was trainined in accordance with the requirements of HCS and 29 CFR 1910.1026: (a) 201 Hancock Boulevard, Reading Pennsylvania - An employee was exposed to chromium (VI) while welding on stainless steel. The employer did not provide training on the chromium (VI) in accordance with the requirements of the standard, on or about April 5, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: (a) 201 Hancock Boulevard, Reading Pennsylvania - An employee that works with propane was not provided with the required training on the changes in the hazard communication program such as labeling, pictograms, and safety data sheets on or about May 5, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
