29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide abatement documentation for the following cited hazards:  a.  Employees were exposed to a fall hazard due to a missing mid-rail on the third story deck on or about May 5, 2014.  b.  Employees were exposed to fall hazards and were not provided training on fall protection procedures.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.    On or about May 5, 2014 employees were exposed to a fall hazard due to a missing mid-rail on the third story deck.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about May 5, 2014 employees who were exposed to fall hazards were not trained in fall protection procedures to recognize and minimize those hazards.
