29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Roof - An employee tearing off shingles of a 7 X 12 pitch roof was exposed to a fall hazard of approximately eighteen (18) feet without the use of any guardrail system, safety net system, or personal fall arrest system, on or about May 09, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 2984 Rumson Drive - The portable extension ladder used to access the roof did not extend three (3) feet above the upper working surface exposing employees to a fall hazard of approximately eighteen (18) feet during access and/or egress, on or about May 09, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) 2984 Rumson Drive - An employee using a portable extension ladder was not instructed in the recognition and avoidance of unsafe conditions exposing him to a fall hazard of approximately eighteen (18) feet from a ladder that was not three (3) feet above the working surface, on or about May 09, 2014.  b) 2984 Rumson Drive - An employee tearing off a roof was not instructed in the recognition and avoidance of unsafe conditions exposing him to a fall hazard of approximately eighteen (18) feet from the roof of a two story building, on or about May 09, 2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
