The equipment listed below contained both electrical and hydraulic energy sources.  Employees exposed to the hazard of being caught in machines and electrical shock hazards.     a. TMC #1, injection molding machine manufactured by TMC Magnetics and Machinery , Serial number 9012065, in shop area 1.  b. Cincinnati injection molding machine #2, manufactured by Cincinnati, model VSX55-2.27, serial 3455A2V92-55, in shop area 1.   c. TMC #3, injection molding machine manufactured by TMC Magnetics and Machinery, Model A-150, serial 9004026, in shop area 1.  d. Cincinnati injection molding machine #4, the model number, Cincinnati VSX33-1.08, serial 3433A2192-19, in shop area 1.   e. TMC #5, injection molding machine in shop area 1.   f. TMC #6, injection molding machine in shop area 1.   g. TMC#7 injection molding machine, model 150E, in shop area 2.    h. TMC #8 injection molding machine, model 250E.

On or about May 9, 2013, in shop #2, the employer failed to ensure the Security Engineer grinder was in good repair. The outer sheathing of the cord was pulled out of the plug, exposing the insulated wires. Employees exposed to an electrical shock hazard.     b. On or about May 9, 2013, in shop #2, TMC Injection machine #9, the employer failed to ensure the outer sheathing of the flexible cord entered the plug. The insulated wires inside the sheathing were exposed. Employees exposed to an electrical shock hazard.
