Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   (a) Location: 101 Main Street, Medford MA, 02155   On or about 5/12/2014, employees were not protected from falling while working on a flat roof greater than 50 feet in width and at a height of more than 6 feet from the lower level.
