29 CFR  1926.501(b)(13): The employer does not ensure that each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are protected by guardrail systems, safety net system, or personal fall arrest system, or provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  Location: 20 Roach Street in Quincy, Massachusetts   Employees performing roofing work are not protected from fall hazards by the use of a fall protection system.    The employer fails to assure effective protections against falls as required by the standard, as demonstrated on December 8th 2022, when employees were working on the roof of a residential structure where employees were not using adequate means of fall protection.  Instance #1 - from a height of approximately 18 feet, from the roofline of the home to the ground below, where personal fall arrest systems was not in use. 29 CFR  1926.502(d)(15): The employer does not ensure that anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached or designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  Location: 20 Roach Street in Quincy, Massachusetts   Employees performing roofing work are not protected from fall hazards by the use of roof anchors installed under the supervision of a qualified person.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on December 8th 2022, when employees were working on the roof of a residential structure where employees were not using adequate means of fall protection.  Instance #1 - from a height of approximately 18 feet, from the roofline of the home to the ground below, where the roof anchors were not installed over the peak of the roof, were not fastened securely to a structural member of the roof, were not installed with the correct fasteners, and were 14 fasteners short of the minimum number of fasteners per the fall protection training provided by Roof Kings. 29 CFR  1926.502(d)(21): The employer does not ensure that personal fall arrest systems are inspected prior to each use for wear, damage, and other deterioration, and/or ensure that defective components are not removed from service:  Location: 20 Roach Street in Quincy, Massachusetts   Employees performing roofing work are not protected from fall hazards by the use of personal fall arrest systems free of damage and defective components.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on December 8th 2022, when employees were working on the roof of a residential structure where employees were not using adequate means of fall protection and personal fall arrest systems were not inspected prior to each use and defective components were not removed from service.  Instance #1 - from a height of approximately 18 feet, from the roofline of the home to the ground below, where the lifeline was tied  with an untested, nonengineered handtied knot to the roof anchor.  Instance #2 - from a height of approximately 18 feet, from the roofline of the home to the ground below, where the lifeline showed obvious signs of wear, damage, and deterioration and the lifeline is connected to a lanyard with an untested, nonengineered handtied knot.  Instance #3 - from a height of approximately 18 feet, from the roofline of the home to the ground below, where the lifeline showed obvious signs of wear, damage, and deterioration.
