29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:   a) 4418 Sterling Pointe Dr., Kennesaw, GA: Employees installing roofing shingles onto an 8:12  and 10:12 pitch roof were exposed to arresting forces and a lower level fall hazard of over 25 - feet, in that; procedures for erecting personal fall arrest systems had not been explained or implemented.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  a) 4418 Sterling Pointe Dr., Kennesaw, GA: The employer did not ensure that a ladder being used to access a roof was extended at least 3 feet above the roof surface. Employees were exposed to a fall hazard of approximately 21- feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.    a) 4418 Sterling Pointe Dr., Kennesaw, GA: Employees installing roofing shingles onto an 8:12  and 10:12 pitch roof were exposed to arresting forces and a lower level fall hazard of over 25 - feet, in that; no means or methods of approved fall protection were in use.   Cowboys Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 901191, citation number 1, item number 1, and was affirmed as a final order on 08/13/2013 with respect to a workplace located at 4839 Kinsbury Drive Smyrna, GA 30127
