29 CFR 1926.502(d)(15)(i):  Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two:   a) The roof of residence located at 2402 Saint Feagin Place, Powder Springs, Georgia. Employees, removing composite shingles from steep, 10 in 12 pitched roofs were exposed to fall hazards of 18-feet 8-inches and 9-feet 9-inches. The anchor point, five eights inch synthetic ropes nailed into the roof, of the inappropriate personal fall arrest systems would not be able to withstand twice the impact load if the employees fell.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   a) The roof of residence located at 2402 Saint Feagin Place, Powder Springs, Georgia. Employees using a rope grab, which was explicitly designed for mountaineering and climbing, as part of a personal fall arrest system were exposed to arresting forces and fall hazards from heights of approximately 18-feet 8-inches and 9-feet 9-inches. The attachment point of the mountaineering belt was not located in the center of the wearers back. The attachment point was positioned to the front of the employee at waist level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) The roof of residence located at 2402 Saint Feagin Place, Powder Springs, Georgia. Employees, who were not wearing personal fall arrest systems, when removing composite shingles from unprotected steep-sloped, 10 in 12 pitch, roofs were exposed to fall hazards of approximately 18-feet 8-inches and 9-feet 9-inches.  Cowboys Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 901191, citation number 1, item number 1 and was affirmed as a final order on 08/13/2013, with respect to a workplace located at 4839 Kinsbury Drive, Smyrna, Georgia.  Cowboys Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 976399, citation  number 1, item number 1 and was affirmed as a final order on 08/1/2014, with respect to a workplace located at 4418 Sterling Pointe Drive, Kennesaw, Georgia.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   a) The roof of residence located at 2402 Saint Feagin Place, Powder Springs, Georgia. Employees were exposed to fall hazards of 20-feet 5-inches, as the side rails of an aluminum extension ladder, used to access the roof, did not extend at least 3-feet above the upper landing.     Cowboys Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 976399, citation number 1, item number 2 and was affirmed as a final order on 08/1/2014, with respect to a workplace located at 4418 Sterling Pointe Drive, Kennesaw, Georgia.
