29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about May 10, 2014, at the above addressed site, a subcontractor employee was exposed to a fall hazard of approximately 34 feet when removing an existing glass wall panel while using an aerial lift without a harness or lanyard attached to a required tie off point.
