29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  At Lot-67 where an employee was installing roof shingles, no conventional or alternative fall protection system was used to protect the employee from a 20-foot fall hazard, on or about May 8, 2014.   Nova Carpentry, Inc. previously doing business as Hugh MacDonald Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1956.501(b)(11), which was contained in OSHA inspection number960026, citation number 01, item number 002 and was affirmed as a final order on 03/27/2014, with respect to a workplace located at406 Winterside Drive, Apollo Beach, FL.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) At Lot-67 where employees used an extension ladder to gain access to the roof of the       two story multi-family unit, the ladder was positioned two rungs above the roofs eave        and not the required minimum of 3 feet or 3 rungs above the roofs eave  which         presented a fall hazard of 20 feet, on or about May 8, 2014.
