On the roof of a residential property a vertical lifeline was secured back to itself using a knot, exposing the employee to a 10 foot fall hazard, on or about 3/25/2015.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a. In the wooden truss section of a roof an employee's lifeline was wrapped around a single 2x4 directly over connection point with another 2x4 and back to lifeline itself, exposing the employee to a 10 foot fall hazard, on or about 3/3/15.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a. On the 4/12 pitched residential roof an employee's lifeline was rigged in a manner that would not prevent the employee from free falling and impacting the lower level, exposing the employee to a 10 foot fall hazard, on or about 3/3/2015.      Hugh MacDonald Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(16)(iii), which was contained in OSHA inspection number 316494657, citation number 1, item number 1 and was affirmed as a final order on 7/5/2012, with respect to a workplace located at 5908 Fishhawk Crossings, Lot 7, Block EE, Lithia, FL, 33569.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a. At the front door the portable extension ladder that was used to gain access to the roof only extended approximately 18 inches past the landing surface exposing employees to a 10 foot fall hazard, on or about 3/3/2015.    Nova Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 976570, citation number 1, item number 2 and was affirmed as a final order on 7/29/2014, with respect to a workplace located at 8725-8737 Via Tavoleria Way, Lots 67-70, Windermere, FL 34786.
