29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:  In the machine shop the Grizzly Vertical Mill did not have a guard. Employees were exposed to the hazards of flying chips and/or contacting the rotating chuck.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  1.    In the Big Building  Panel B breakers 7, 9, 11, 13, 15 17 and 20 were not labeled. Breakers 12, 14 , 16 and 18 were not labeled on the Legend but did have partial labels on the panel cover. Breakers 19 and 21 had conflicting labels on two different locations. Breakers 23 and 25 had conflicting labels on two different locations.  2.    In the Big Building  Panel D breakers 31 through 42 were not labeled on the Legend. 3.  In the West Building an unidentified breaker panel on the east wall of the South Facility had the Legend of Breakers missing and the labeling located on the panel itself was incomplete and/or illegible.  Employees were exposed to electrical shock and/or struck-by/caught between hazards.

29 CFR 1910.303(f)(3): The markings required by paragraphs (f)(1) and (f)(2) of 29 CFR 1910.303 were not sufficiently durable to withstand the environment involved:  1. In the Big Building Panel B and Panel D breakers were not labeled in a manner that was durable and legible.   2. In the West Building an unidentified breaker panel on the east wall of the South Facility did not have the breakers labeled in a manner that was durable and legible.  Employees were exposed to a shock hazard and/or a struck-by/caught between hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.    In the East Building at Blow Molding Machine 2, Conveyer C-17 was being provided electrical power via a yellow AWG-12 extension cord. Employees were exposed to a potential shock hazard.
