29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks, unused portion of saw blade: The two band saws located in the assembly area that are used to cut the plastic bollards to length did not have the unused portion of the blade/s enclosed/guarded when the 4" and 7" units were being cut to length. Employees were exposed to the hazard of contacting the moving blade.

On the manufacturing floor in the Main Plant - oil residue and two puddles of oil were located in the near vicinity of the oil storage/use facility. Employees were exposed to a slip/trip hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to the joints or terminal screws: In the assembly area the electrical cord used to power the ITW MIMA Wrapper lacked the proper strain relief on the female plug end.
