29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) 888/890 Huguenot Avenue, Staten Island, NY 10312. The employer, along with his employee were working on and from tubular scaffold doing stucco work when the walking/working platforms of the scaffold were not fully planked exposing them to fall hazards in excess of 13 feet to the ground below. Condition occurred on or about 05/13/14.

The employer, along with his employee were working on and from tubular scaffold doing stucco work with no fall protection systems in place, i.e. guardrails installed along all open sides and ends of the walking/working platforms exposing them to fall hazards in excess of 13 feet to the ground below. Condition occurred on or about 05/13/14.

29 CFR 1926.1053(b)(1): Portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) 888/890 Huguenot Avenue, Staten Island, NY 10312. The employer along with his employee were working on and from tubular welded scaffold with no safe access to the walking/working platform in that the 16 foot Base Section of aluminum portable extension ladder they were using was not secured at its top to a rigid support and a grabrail was not provided to assist employees in mounting and dismounting the ladder. Condition occurred on or about 05/13/14.
